HIPAA authorizations

Contact providers with the death certificate; HIPAA still governs who may receive records after death in many cases

How

Ask each provider's medical records department what they require to release records after death. As personal representative of the estate, you generally have the right to request records, so say so explicitly. Submit a written request naming the records and the date range you need. Request records only for a specific purpose, such as an insurance claim or a legal question.

A certified copy of the death certificate Letters testamentary or proof you are the personal representative
